Release Forms

The legal next of kin must print and complete the Autopsy Release Form and return the form via email to lvautopsy@gmail.com.

If your loved one is at a Palm Mortuary (i.e., Neptune, King David, Thomas and Jones, Palm, or Affordable Cremation), please also complete the Release Authorization Palm Mortuary.
